Mossberg 535 with a Mueller quick shot sight.

My pair of Mossberg 835's.  The pistol grip is my primary turkey gun and the factory stock is my backup and everything else gun.  They have Tru Glo dual color multi retical reflex sights mounted on Mossberg Picatinny rails.  The pistol grip stock is ATI and both have the Brownells raised steel safety button kits.  Both guns are custom dipped in APG and both have custom made side attached two point tactical slings.  Lastly the pistol grip has an Indian Creek .675 and the other an Indian Creek .710 which throws a wicked buckshot pattern.

Is that the earlier discontinued  Bushnell model, or an actual "EoTech" model?

That's an older Bushnell. Bushnell will not service their HoloSight although their website last year said they did, These were made for Bushnell by EOTech and they can service certain units if you persist. Mine needed a Nitogen recharge and rear screen because it's delaminating. Hopefully EOTech has the screen.
