That cap was made by Russell Athletics (Mossy Oak Apparel) back in 2007.  You might be able to find that exact model on ebay, but the Mossy Oak store has some logo caps very similar.

The MO store has the green leaf with the old tree logo. I got one last year..

I think that I saw one like the one in the picture or close to it in Wal-Mart and Academy Sports
