I filled it out last year received my 2016 gobbler report in the mail today and my 2017 survey also.

Yes they want info one gobbling activity, turkey numbers, predator numbers, timber in activity ect. They claim to be doing this survey for years but I believe this is the first I have seen it. I have already filled up the comments section and will be attaching a letter. Really happy to see they are at least asking for input.
