Here is The Roaring 20's Turkey Scoring Sheet saved in .jpg and .pdf formats.

What you can do is print off a few of them and fill them in with black marker (like a Sharpie) in large letters once you have determined the scoring statistics on your bird. Along with your measurement photos, make sure that you include a clearly legible photograph of yourself, this filled-out scoring sheet, and the recently vanquished, feathered antagonist.
